    Review Spam Detection Using Machine Learning Techniques

    Nowadays with the increasing popularity of internet, online marketing is going to become more and more popular. This is because, a lot of products and services are easily available online. Hence, reviews about these all products and services are very important for customers as well as organizations. Unfortunately, driven by the will for profit or promotion, fraudsters used to produce fake reviews. These fake reviews written by fraudsters prevent customers and organizations reaching actual conclusions about the products. Hence, fake reviews or review spam must be detected and eliminated so as to prevent deceptive potential customers. In our work, supervised and semi-supervised learning technique have been applied to detect review spam. The most apt data sets in the research area of review spam detection has been used in proposed work. For supervised learning, we try to obtain some feature sets from different automated approaches such as LIWC, POS Tagging, N-gram etc., that can best distinguish the spam and non-spam reviews. Along with these features sentiment analysis, data mining and opinion mining technique have also been applied. For semi-supervised learning, PU-learning algorithm is being used along with six different classifiers (Decision Tree, Naive Bayes, Support Vector Machine, k-Nearest Neighbor, Random Forest, Logistic Regression) to detect review spam from the available data set. Finally, a comparison of proposed technique with some existing review spam detection techniques has been done

    Magnetic field driven enhanced ferroelectric switching in self-grown ferroelectric-ferromagnetic composite in the BiFeO3-BaTiO3 multiferroic alloy system

    Over the years attempts have been made to compensate for the inherent weaknesses in the bulk state of the multiferroic BiFeO3, such as high leakage current and the absence of ferromagnetic correlation, and exploit its magnetoelectric potential by forming solid solutions with other perovskites. Studies in the recent few years have shown that alloying of BiFeO3 with BaTiO3, both with and without additives, can induce both ferroelectric and ferromagnetic switching. While the coexistence of both the ferroic orders is encouraging from the view point of technological applications, the origin of ferromagnetism in this system remains elusive. Here, we synthesized such compositions and carried out a detailed structural analysis employing magnetic separation of the powder particles. We found that the origin of ferromagnetism lies in the spontaneous precipitation of a minor ( ~ 1 wt %) barium hexaferrite (BaFe9O19) phase, often undetected in routine x-ray diffraction studies of powders sampled from the entire specimen. We also demonstrate that inspite of the insignificant fraction the ferrimagnetic phase, this self-grown composite exhibit noticeably enhanced ferroelectric switching in the presence of external magnetic field. We obtained a dc magnetoelectric coupling of ~ 9 x 10-8 s/m, a value which is comparable to what has been reported for layered ferroelectric/ferromagnetic laminates and bilayer thin film ferroelectric-ferromagnetic hetrostructures. Our study suggests that reasonably large magnetoelectric coupling is realizable in simple 0-3 ferroelectric-ferromagnetic bulk composites provided synthesis strategies are developed which induces spontaneous precipitation of the ferromagnetic phase in small volume fraction to ensure good insulating behaviour of the composite thus developed.Comment: 21 pages, 7 figure

    A study of reporting pattern of adverse drug reactions in a tertiary care teaching hospital

    Background: Adverse drug reactions (ADRs) are one of the prime causes of morbidity and mortality, increase in hospital stay and socioeconomic burden on the patients. Periodic monitoring aids in formulating methods for safe usage of medicines in hospitals. Identification of ADRs and their reporting pattern can provide useful information for their prevention. Hence this study was done to see the pattern of reported ADRs in Patna Medical College and Hospital, Patna in a 3 months of study.Methods: It was an observational and retrospective study carried out between July 2022 to September 2022. Both outpatients and inpatients were included in the study. The ADRs in the form of Individual Case Safety Reports (ICSRs) were sent to the nearby adverse drug reaction monitoring centre (AMC).Results: The occurrence of ADRs was more common in females (56.25%) as compared to males (43.75%). Patients of age-group 21-40 years (40.625%) were most commonly involved. Medicine department (34.375%) reported the maximum percentage of ADRs. Antimicrobials (37.50%) was the most common drug-group causing ADRs. Maximum reported ADRs (81.25%) were probable, 9.375% were possible, 6.25% were certain, while 3.125% were unlikely with the suspected drug as per Naranjo scale.Conclusions: The pattern of ADRs reported in our hospital is comparable with the results of studies conducted in hospital setup elsewhere, along with a few differences. The study results revealed opportunities for interventions in ADR management especially for the preventable ADRs to ensure safer drug use

    Save The King: Human-King Cobra, Ophiophagus hannah (Cantor 1836), conflicts and the need for conservation strategies in Nepal

    Abstract.—Snake research tends to have a low priority in Nepal and very little information, mostly confined to populationsin small areas, addresses the biology and threats to the King Cobra (Ophiophagus hannah). Herein we providedata that could facilitate an assessment of the species’ status in Nepal and begin to address its conservation needs.We recorded data on King Cobras from 2015–2020, crosschecking with previous studies to avoid any duplicationof records. A King Cobra from Siddhara, Arghakhachi District, was the first record of the species in the district. Werecorded a total of 50 King Cobra mortalities from 20 districts, with most of them killed near human settlementsadjacent to forested areas. We mapped the locations of all mortalities and recorded land-use changes within a 500-mbuffer around each site over a 30-year period (1990–2020), revealing extensive landscape fragmentation in previouslyconnected natural areas. Our data suggest that the major threats to King Cobras are deliberate killing by humans andlarge-scale habitat loss due to an increasing human population. We recommend increased research to better understandthe biology of this charismatic species and continued conservation education and community outreach programs tofacilitate the development of effective conservation strategies

    A seven year experience in patients undergoing total thyroidectomy for benign diseases in a single unit of tertiary care centre: a retrospective case study

    Background: Thyroidectomy is a common surgical procedure performed worldwide by surgeons with varied training. It is the experience and the skills by which a surgeon performs, the indication of surgery and the extent of surgery which determines the outcome and the complication rates.Method: This study is a retrospective study performed from January 2012 to December 2018 when a total of 170 patients underwent total thyroidectomy via kocher’s incision for benign diseases with clinically in hypothyroid state at Safdarjung Hospital, New Delhi in a single unit of a tertiary care hospital.Results: The male: female ratio was 1:7.09 with average age of patients being 40years. The overall complication rate was 18.82% (32 patients). Postoperative transient hypocalcaemia being the most common complication. There was no case of permanent hypocalcaemia. There was no injury to any nerve (recurrent or superior laryngeal). Transient recurrent laryngeal paresis was noted in 4(2.35%) which resolved with conservative management. Complications like wound infection, sympathetic chain injury were not noted. The characteristic feature of this study was five cases of giant secondary retrosternal goitre which had average weight of greater than 500 grams which were operated using kocher’s incision. The average weight of other 165 patients was 70grams +/- 12 grams. There were no major post-operative complications except for post-operative recurrent laryngeal nerve paresis and seroma formation which was evacuated after 3 weeks of surgery.Conclusion: Hence, from this it is concluded that it is the surgical experience and skills of the surgeon which make total thyroidectomy a safe procedure providing permanent cure with a low post-operative morbidity risk even with large sized retrosternal thyroid glands which can be operated via neck incision

    Adverse drug reactions due to gabapentin-nortriptyline in patients with neuropathic pain in a tertiary care teaching hospital

    Gabapentin and Nortriptyline are used to treat neuropathic pain. Gabapentin belongs to a group of drugs called anti-convulsant and Nortriptyline belongs to tricyclic antidepressant. Gabapentin acts by binding to the specific site on voltage- gated calcium channels. This helps in relieving nerve pain and lowers the risk of seizure. Nortriptyline works by preventing the released chemical messengers in brain (serotonin and norepinephrine) from being reabsorbed back into the nerve cells. As a result, it prolongs the effect of serotonin and norepinephrine in the brain and over time, this helps to relieve depression and lighten the mood. We are reporting a case series including 3 patients having neuropathic pain and taking tablet gabapentin-nortriptyline in department of neurology, Patna medical college and hospital, Patna developed some adverse drug reactions. Causality assessment was done in each case by Naranjo scale. Each case was filled properly in ADR (adverse drug reaction) reporting form and was sent to nearby adverse drug reaction monitoring centre (AMC). During the posting for pharmacovigilance in neurology department of Patna medical college and hospital, Patna from 01/07/2022 to 30/09/2022 we got three adverse drug reactions due to gabapentin-nortriptyline. Two of them were due to Gabapentin and one was due to nortriptyline. Gabapentin-nortriptyline is one of the most commonly used drugs in neurology department. It causes some frequent as well as some rare adverse drug reactions which are required to be assessed and finally reported to the nearby AMC

    Insulin Augmentation of Glucose-Stimulated Insulin Secretion Is Impaired in Insulin-Resistant Humans

    Type 2 diabetes (T2D) is characterized by insulin resistance and pancreatic β-cell dysfunction, the latter possibly caused by a defect in insulin signaling in β-cells. We hypothesized that insulin’s effect to potentiate glucose-stimulated insulin secretion (GSIS) would be diminished in insulin-resistant persons. To evaluate the effect of insulin to modulate GSIS in insulin-resistant compared with insulin-sensitive subjects, 10 participants with impaired glucose tolerance (IGT), 11 with T2D, and 8 healthy control subjects were studied on two occasions. The insulin secretory response was assessed by the administration of dextrose for 80 min following a 4-h clamp with either saline infusion (sham) or an isoglycemic-hyperinsulinemic clamp using B28-Asp-insulin (which can be distinguished immunologically from endogenous insulin) that raised insulin concentrations to high physiologic concentrations. Pre-exposure to insulin augmented GSIS in healthy persons. This effect was attenuated in insulin-resistant cohorts, both those with IGT and those with T2D. Insulin potentiates glucose-stimulated insulin secretion in insulin-resistant subjects to a lesser degree than in normal subjects. This is consistent with an effect of insulin to regulate β-cell function in humans in vivo with therapeutic implications

    Comparison of urodynamic parameters with respect to neurological levels in post-traumatic spinal cord injury patients

    Background: Urodynamic evaluation is mandatory in order to correctly assess and classify bladder dysfunction in spinal cord injury (SCI) patients. Study investigated patterns of neurogenic bladder dysfunction in patients with post traumatic spinal cord injury and assessed the relationship of detrusor leak point pressure with compliance, post void residual urine volume and maximum cystometric capacity.Methods: Eighty six patients with neurogenic bladder secondary to traumatic spinal cord injury (SCI) underwent cystometry with electromyography (EMG). T-test was used to compare detrusor leak point pressure (LPP) between complete and incomplete injury groups. Pearson correlation test was used to seek correlation between detrusor LPP and compliance, post void residual volume (PRV) and maximum cystometric capacity (MCC).Results: Mean detrusor LPP in suprasacral complete injury group, suprasacral incomplete injury group and sacral complete injury was 52±21 cm of H2O, 53±18 cm of H2O and 16±9 cm of H2O respectively. No significant difference in detrusor LPP was found between suprasacral complete and incomplete group on t-Test (p= 0.571068). Significant difference in detrusor LPP was found between suprasacral and sacral group (p= 5.71891E-12). Mean compliance in sacral injury group was 24±16 and in suprasacral complete injury group was 5±6. Mean compliance in suprasacral incomplete injury group was 4±2. Pearson correlation showed negative correlation (r = -0.6918934) between detrusor leak point pressure and compliance (p= 1.2744E-13). Negative correlation (r = -0.311409922) was observed between detrusor leak point pressure and post leak/ void residual urine volume (p= 0.003335033) and between detrusor LPP and maximum cystometric capacity (r = -0.31354), (p= 0.003115).Conclusions: Significant difference in urodynamic parameters exists between sacral and suprasacral injury patients. However there is no significant difference in urodynamic parameters between complete and incomplete injury at suprasacral level

    A case of drug induced oral erythema multiforme

    Erythema multiforme (EM) is an acute, self-limited, usually mild, and often relapsing mucocutaneous syndrome. It occurs predominantly in younger age group with slight male preponderance. The classical cutaneous target lesions and mucosal bullae or erosions are clinical manifestations of EM. Drug induced oral EM a rare clinical entity which involves only the lips and oral mucosa without skin involvement. These lesions are difficult in diagnosing with other oral ulcerative lesions with similar clinical manifestations. Here we are reporting a case of phenylbutazone induced oral EM. The causality of phenylbutazone in the reaction was “probable” as per Naranjo scale and seriousness of the reaction was “prolonged hospitalization”. The patient was managed by immediate withdrawal of the suspected drug along with conservative managements to prevent secondary infections. The patient was discharged successfully after 15 days of admission

    Closing the Feedback Loop: Collaborative Design of a Musculoskeletal Revision Course

    Clinical Teaching Fellows (CTFs) at Leicester Medical School decided to supplement the learning needs of first year students on the musculoskeletal (MSK) module. Mid-unit evaluation had demonstrated that students had remarked positively on CTF teaching and many requested further anatomy teaching and demonstrations using prosections. It is increasingly important to provide students with evidence that their feedback is being acted upon, therefore CTFs collaborated in providing additional learning resources in the form of CTF-led revision courses while the MSK module was still ongoing. A survey was designed which aimed to engage students and to further explore their learning needs when developing the course. Based on these responses, two half-day CTF-led revision courses were designed, which included educational methods and topics the students themselves had suggested. CTFs collaboratively developed eight different stations, with one CTF designing and delivering the teaching material. Attendance was high and feedback indicated this was a valuable learning experience for students, with particularly positive responses about the interactive nature and high quality of the teaching. This experience demonstrates the benefits of working in partnership with students when developing learning activities, closing the feedback loop to improve student satisfaction, and collaborative planning when designing revision resources
